Before participating in your first studio training session, please complete my digital liability waiver.

For adult studio participants age 18 and older.

You can review and sign on your phone before your session. No printing or scanning is necessary. Please use your full legal name and the same email address associated with your booking. The signing page opens in a new tab.

This adult studio waiver is for participants age 18 and older. A parent or legal guardian must complete a separate youth agreement for a participant under 18.

I use Jotform Sign to keep signed waivers and their audit records privately. Signed waivers are not posted on this website.
